EDUCATIONAL OBJECTIVES
At the completion of this webinar, pharmacy students will be able to:
- Explain the process of medication reconciliation.
- Understand why medication reconciliation is important for patient safety.
- List the most common barriers to effective medication reconciliation.
- Identify strategies to improve the medication reconciliation process.
