This program will educate pharmacy students on the process of medication reconciliation and its importance to patient safety.

EDUCATIONAL OBJECTIVES

At the completion of this webinar, pharmacy students will be able to:

  1. Explain the process of medication reconciliation.
  2. Understand why medication reconciliation is important for patient safety.
  3. List the most common barriers to effective medication reconciliation.
  4. Identify strategies to improve the medication reconciliation process.
